I can almost guarantee it's the MediaKit beta that is causing the trouble. They changed something.
As I won't touch the leaked MediaKit your on your own.
I can tell you (or anyone else) that it's VolumeSlider.cpp in the sourcecode that tries to find and create the controller. I don't know if it's just minor changes or the whole file that needs fixing for the new MediaKit.

I'm using BeOS 5 Pro, OT from CVS (maybe that's what's causing it) and the MediaKit beta (leaked).

Weird, the latest version has been developed / tested with SB-Live.
What version of BeOS are you running, stock R5.0.x or are you using some leaked/3rd party components that could cause problems?
Seems like I have to improve soundcard-compability.

Can you control the volume if you start it as an ordinary application?
Also I'd like to know what soundcard you have and what the main volumecontrol on the [soundcard] out-tab is called. (usually Mixer).

I'm using the latest OT from CVS (5.1.9) and inf I add VolumeControl to the Deskbar with the -deskbar flag it makes the Deskbar disappear.
